- Provide assessments/referrals (i.e. shelters, emergency food, 302 commitments)
- Provide assessments/referrals (i.e. RC/ICM, CRR, RTF, Hedwig House, PHP, consumer center, Wraparound, transportation)
- Provide Code Blue/Code Red
- Provide crisis intervention
- Conduct home visits/outreach
- Track OP/PHP court orders and facilitate 306 petitions when appropriate
- Coordinate/attend treatment team meetings
- Assist with housing issues (i.e. rent rebates, HAP, 811 housing program)
- Assist with entitlement access (Medical Assistance, Food Stamps, Social Security)
- Maintain daily logs and progress notes on undefined caseload
- Track and assign referrals
- Attend community meetings to enhance knowledge of community resources
- Attend available trainings.
- A Bachelor's Degree with major course work in sociology social work, psychology, gerontology, criminal justice, theology, nursing, counseling or education.
- A High School Diploma and twelve semester credit hours in fields of study previously identified and two year's experience in public or private human services with one year in direct client contact.
- High School Diploma and five years of mental health direct care experience in public or private human services.
- Must have a valid Driver's License
