Olathe Medical Center, Inc, a 300-bed general acute care facility, is currently looking for a Social Worker to join our team.
Position Summary:
Provides professional social work services in the acute care hospital setting, utilizing supportive counseling thru biopsychosocial assessments. Implements clinical knowledge in assessing the needs of patients and families during illness. Provides crisis intervention as patients and families transition to post discharge planning. Collaborates with members of interdisciplinary health care team in the development of a transition plan and addresses needs in a timely manner. Participate in program planning and development to meet the needs of customers within the health care system and community.
Position Requirements:
Education:
Graduate Degree, Masters degree in Social Work or LMSW from Accredited program required
Experience:
Any Job Related Experience
Previous Healthcare Experience, Discharge planning, case management, or social work in a acute care medical hospital - Preferred
Knowledge, Skills:
Organizational, Customer Relations, Verbal Communication -Required
Ability to Multi Task and Prioritze - Preferred
License, Registration and/or Certification:
License to practice Social Work in the state of Kansas - Upon Hire
